Polymyalgia reumatika och temporalisarterit

Definition:
Polymyalgia reumatika (PMR) är ett kliniskt syndrom hos äldre som karakteriseras av värk och stelhet i skulder- och/eller bäckenområden, ofta med allmänsymtom. PMR och temporalisarterit förekommer ibland tillsammans. 
Förekomst:
Årlig incidens av PMR bland personer ≥50 år är 58–96 per 100 000 och för temporalisarterit cirka 20/100 000. Kvinnor drabbas 2–3 gånger oftare än män.
Symtom:
Huvudsymtom vid temporalisarterit är huvudvärk och synpåverkan. Mest framträdande symtom vid PMR är stelhet och smärta i proximal muskulatur, oftast symmetriskt i skulder- och bäckengördel, överarmar och lår.
Kliniska fynd:
Få typiska fynd vid PMR, men ofta finns ömhet i muskulaturen. Temporalisarterit kan ha lokaliserad ömhet vid temporalisartären, som kan vara förtjockad.
Diagnostik:
SR och CRP är ofta förhöjda. Temporalisbiopsi vid misstänkt temporalisarterit.
Behandling:
Kortikosteroider – vanlig startdos är 15–30 mg dagligen, men högre doser ges vid temporalisarterit. Supplement med kalcium och D-vitamin och eventuellt benspecifik behandling för att motverka sekundär osteoporos.
