Förstoppning hos barn

Definition:
>95 % av barn med förstoppning över ett års ålder har funktionell förstoppning. Diagnostiska kriterier för funktionell förstoppning hos barn finns (Rom IV).
Förekomst:
Upp till en tredjedel av alla barn i åldern 6–12 år rapporteras någon gång ha varit förstoppade.
Symtom:
<3 kompletta tarmtömningar per vecka, hård avföring, ofrivilligt avföringsläckage, nedsatt aptit som förbättras efter tarmtömning, magsmärtor, färskt blod efter analfissur.
Kliniska fynd:
Vid funktionell förstoppning – mjuk buk, eventuellt palpabel utfyllnad motsvarande kolon med avföring i nedre vänstra kvadranten, eventuellt med soiling.
Diagnostik:
Typisk klinisk bild, eventuellt serologi för celiaki och blodprover för hypotyreos.
Behandling:
Information om patofysiologiska mekanismer bakom funktionell förstoppning, vikten av att inte "hålla sig" och bra avföringsvanor. Vid uttalade och långvariga besvär övervägs läkemedelsbehandling. Remiss till barnläkare vid misstanke om organisk orsak till förstoppning (utöver analfissur).
