Dupuytrens kontraktur

Definition:
Dupuytrens sjukdom är ett tillstånd med gradvist ökande fibros i palmarfascian.
Förekomst:
Framförallt hos män, och ökande förekomst med åldern. I en isländsk population fanns tillståndet (med kontraktur av finger/fingrar) hos 12 % av 70–74-åriga män.
Symtom:
Börjar vanligen som en smärtfri knuta i handflatan som efter hand leder till strängbildning och senare kontraktur. 
Kliniska fynd:
Kontrakturer oftast av ring- och lillfinger, men samtliga fingrar kan involveras.
Diagnostik:
Typisk klinisk bild. Inga kompletterande undersökningar behövs.
Behandling:
Aktiv behandling är i första hand nålfasciotomi. Kollagenasinjektion har samma behandlingseffekt som nålfasciotomi, men är dyrare. Vid mycket svåra extensionsdefekter kan öppen kirurgi behövas.
