Gallstenssjukdom och akut kolangit

Definition:
Gallstenssjukdom beror på gallsten i gallblåsan eller i de djupa gallgångarna. Kan ge kolangit, bakteriell infektion i gallgångar, kolecystit och pankreatit.
Förekomst:
De flesta personer med gallstenar är asymtomatiska, prevalensen ökar med stigande ålder.
Symtom:
Ofta smärta över gallblåseområdet. Ikterus förekommer vid ocklusion. Vid kolangit uppstår sjukdomskänsla och feber.
Kliniska fynd:
Typiskt vid symtomatisk sjukdom är återkommande smärtor över höger övre bukhalvan. Ikterus förekommer vid total ocklusion och vid kolangit. Vid kolangit finns tecken på systemisk infektion/sepsis.
Diagnostik:
Diagnosen gallstensanfall baseras på symtom, kliniska fynd och ultraljudsundersökning. Kolangitdiagnosen baseras på tecken på systemisk infektion, kolestas och positiva fynd vid bilddiagnostik.
Behandling:
Symtomlindring. Vid symtomatisk gallstenssjukdom rekommenderas operation, i vissa fall observation, beroende på lokalisationen av stenen. Vid kolangit ges antibiotika. Akut opereation kan behövas för att ta bort obstruktion i gallgångar.
