Hepatit C

Definition:
Inflammation i levern orsakad av hepatit C-virus (HCV). HCV-infektionen blir kronisk hos cirka 60–80 %. Sprutnarkomaner är den grupp som är mest utsatt. I cirka 45 % av fallen är smittvägen okänd.
Förekomst:
I Sverige bedöms cirka 35 000–45 000 personer ha en akut eller kronisk hepatit C-infektion. Globalt beräknas 71 miljoner människor ha kronisk hepatit C, motsvarande 1 % av världens befolkning.
Symtom:
Den akuta fasen av infektionen är vanligtvis subklinisk (cirka 70 %) eller har ett lindrigt kliniskt förlopp. Patienter utan symtom spontanläker sällan och när infektionen blir kronisk fortgår tillståndet vanligtvis länge utan symtom. Efter ett varierande antal år kan HCV-infektionen leda till cirros, leversvikt och primär levercancer.
Kliniska fynd:
Vanligen inga patologiska kliniska fynd. Vid akut symtomatisk hepatit C ses ofta ikterus och ibland svullen, ömmande lever. I sent skede, vid levercirros, kan kliniska fynd talande för portal hypertension och leversvikt uppstå (spider naevi, förstorad lever, ascites, esofagusvaricer).
Diagnostik:
Diagnosen bekräftas med virologisk diagnostik (anti-HCV, HCV-RNA).
Behandling:
Kombinationer av nya direktverkande antivirala läkemedel – i grupperna polymerashämmare, proteashämmare och NS5A-hämmare.
