Erysipeloid

Definition:
Bakteriell zoonos som orsakas av Erysipelothrix rhusiopathiae som leder till en självbegränsande, röd svullnad på smittad hud.
Förekomst:
Sällsynt tillstånd som förekommer främst hos slaktare, fiskare och veterinärer. Immunsupprimerade har beskrivits kunna utveckla sepsis i sällsynta fall.
Symtom:
Lokaliserat eller diffust hudutslag, eventuell systemisk infektion. Infektionen är vanligen lokaliserad till fingrar eller händer.
Kliniska fynd:
Erysipeloid – röd svullnad i huden, ofta rosafärgad centralt och med upphöjd kant.
Diagnostik:
Diagnosen ställs på typisk anamnes och klinisk bild. Kompletterande undersökningar tillför sällan något. För positiv odling eller PCR krävs ett djupt liggande prov, exempelvis via biopsi. Blododlingar är positiva vid sepsis.
Behandling:
Penicillin V, alternativt cefalosporiner.
